Verwijzing

Verwijsbrief van uw huisarts

Om een behandeling bij onze praktijk vergoed te kunnen krijgen, dient u een geldige verwijzing van uw (huis)arts of medisch specialist ten tijde van uw aanmelding in bezit te hebben.

Hierop moet type, naam, adres, handtekening (plus praktijkstempel) en de AGB-code van de verwijzer vermeld staan evenals uw naam, adres, geslacht, geboortedatum en polisnummer/zorgverzekeraar (uzovi-nummer). Ook dat er een vermoeden is naar een DSM-benoemde stoornis en dat het een verwijzing betreft naar de generalistische basis-ggz (gb-ggz) of de specialistische ggz (s-ggz), ook wel gespecialiseerde ggz (g-ggz) genoemd.

Dan is het van belang dat op de verwijsbrief de verwijzing naar specifiek onze praktijk vermeld staat alsook de datum en dat deze datum niet meer dan 6 maanden ligt voordat u zich bij ons aanmeldt. Het is belangrijk dat u een geldige verwijzing in bezit hebt wanneer u contact opneemt met onze praktijk. Slechts dan kan een eventuele intakeprocedure verder worden opgestart.
